This terrible noise is the result of an aftermarket (likely Gates) timing belt that reacted with the yellow-metal (brass?) coated timing cogs (a toothed pulley turned by a toothed belt). The sound, while it may resemble a bad bearing or grinding, is actually the belt sticking to the cogs then snapping free. Replacing the belt with a toyota OEM (made by MitsubOshi [not a typo/not mitsubishi]) only helps for a while. The cogs them self ($50 per but a pain in the butt) need to be changed and a new belt used
